Matthew J. Beelen, M.D.
Dr. Beelen graduated from the University of Michigan Medical School and completed his family practice residency at Lancaster General Hospital in 2002. He simultaneously completed Faculty Development Fellowships at Lancaster General Hospital and Michigan State University. Dr. Beelen completed his geriatric fellowship in 2005 and has his Certificate of Added Qualifications in Geriatrics. He is currently the Director of the Geriatric Fellowship program and one of the Associate Directors of the Family Medicine Residency Program. His clinical and scholarly interests include: Comprehensive Geriatric Assessment, Inpatient Geriatric Consultation, Dementia, and Geriatric Curriculum Development.

J. Kenneth Brubaker, M.D., C.M.D.
Dr. Brubaker, board certified in family medicine, completed a post graduate fellowship at the Philadelphia Geriatric Center, and has a Certificate of Added Qualifications in Geriatrics. He graduated from Hahneman University and completed his family medicine residency program at Lancaster General Hospital. Dr. Brubaker was in private practice from 1973 until joining the fellowship faculty in 2001. Clinical interests include assessing decision-making ability of patients with dementia. In collaboration with the Masonic Village, he has developed a decision-making tool that is used within the nursing home setting. He also has developed significant expertise serving as a medical director of nursing facilities.

R. Scott DeLong, MD
Dr. DeLong received his medical degree from Pennsylvania State University College of Medicine. He completed a three year family medicine residency at Lancaster General Hospital in 2007. He also completed his Geriatric Fellowship in 2008 and is eligible for his Certificate of Added Qualifications in Geriatrics.

Robert M. Howse Jr. M.D.
Dr. Howse graduated from University of Maryland Medical School and completed the Lancaster General Hospital Family and Community Residency Program. He is board Certified in Family Medicine and has a Certificate of Added Qualifications in Geriatrics. He taught at the Penn State University The Good Samaritan Hospital Family Practice Residency Program until 2005. He practices at Willow Lakes. His area of interest is capacity evaluation.

Dale K. Hursh, M.D., C.M.D.
Dr. Hursh received his doctor of medicine degree at Jefferson Medical College, Philadelphia in 1989. He finished a three year family medicine residency at Lancaster General Hospital in 1992, and practiced primary care medicine in Lancaster County until 2001. Dr. Hursh completed a geriatric fellowship in 2002, and has his Certificate of Added Qualifications in Geriatrics. He is also a Certified Medical Director. Dr. Hursh’s clinical interests include geriatric long-term care medicine, medical direction in long-term care, and medical education.

Leon S. Kraybill, M.D., board certified in family practice, received his medical degree at Temple University School of Medicine. He completed his family practice residency at Lancaster General Hospital in 1988. Dr. Kraybill completed his geriatric fellowship in 2004 and has his Certificate of Added Qualifications in Geriatrics.

Kristen M. Nebel, D.O.
Dr. Nebel received her medical degree from Philadelphia College of Osteopathic Medicine. She completed a three year family medicine residency at Lancaster General Hospital in 2006. She also completed her Geriatric Fellowship in 2007 and holds her Certificate of Added Qualifications in Geriatrics. Dr. Nebel enjoys helping those diagnosed with dementia, as well as their families, to better understand the anticipated changes and ensure that their needs and wishes are met.

Neelofer Sohail, M.D.
Dr. Sohail received her medical degree in Karnataka-Sri Adichunchanagiri Institute of Medical Sciences in Karnataka, India. She completed her residency in family practice at Conemaugh Medical Center, Johnstown, Pa, practicing as Chief Resident. Dr. Sohail worked for one year in a local private practice, and completed her geriatric fellowship in 2006. She is board certified in family practice and has her Certificate of Added Qualifications in Geriatrics. Dr. Sohail loves to work with the older patient population and their families with various medical and social issues.

Christi Ann Stewart, M.D.
Dr. Stewart received her medical degree from the University of Virginia School of Medicine in Charlottesville, Virginia. She completed her family medicine residency at Lancaster General Hospital in 2006. She also completed her Geriatric Fellowship in 2007 and holds her Certificate of Added Qualifications in Geriatrics. Due to Dr. Stewart’s interest in care at the end of life, she is currently completing a fellowship in Palliative Care.

Jane Miller, RN, M.S.N., C.R.N.P.
Jane graduated from Lancaster General Hospital School of Nursing in 1976 and has extensive experience in community/home health, hospice and palliative care. She received her Bachelor of Science in nursing from Millersville University in 1986 and Masters of Science in nursing in 2004. Jane has been board certified as a family nurse practitioner since 2004.

Lesetta Weaver, R.N., M.S.N., C.R.N.P.
Lesetta graduated from Hesston College in 1978 with an Associate Degree in Nursing, and from Eastern Mennonite University in 1998 with a B.S. double major in Management and Organizational Development and Nursing. In 2003, she completed a Master of Science Degree in Nursing and Family Nurse Practitioner Program from Widener University, Chester, PA. She is accredited in Family Practice by the American Academy of Nurse Practitioners.
