I was born and raised on the Caribbean island of Antigua. I migrated to the United States to attend college and received my BS in Biology from Liberty University in Lynchburg, Virginia. I attended Virginia Tech for graduate school in Human Nutrition, and Eastern Virginia Medical School for my medical degree. Prior to going to medical school, I worked as a registered dietitian and diabetes educator for Carilion Health System in Roanoke, Virginia. In my spare time I enjoy reading, hiking and spending time with my husband, Julian, and my daughter, Vivienne. I look forward to becoming a part of the Lancaster community, and I am excited about participating in the rich tradition of Family Medicine at Lancaster General Hospital.

Michelle A. Cardona, MD, MPH

I grew up in Nyack, NY, a suburb of New York City in the Hudson Valley. Since leaving for college, many locations have been home including Worcester, MA, Liege, Belgium, Philadelphia, PA, and most recently Baltimore, MD. The charm of Lancaster and the strength of the residency program drew my husband and me back closer to his hometown of Shrewsbury, PA. While at Jefferson, I became interested in taking care of individuals as part of a family unit and in preventive medicine. During my medical training, I decided to take one year to further my education in public health and increased my interest in health promotion and behavioral change. Aside from medicine, my husband I enjoy hiking, biking, traveling, and spending time with family and friends.

Taylor B. Chambers, DO

I was born and raised in northern-central PA near the city of Williamsport. My first introduction to Lancaster County came while attending Elizabethtown College, where I met my husband and many great friends. After attending medical school in Philadelphia at the Philadelphia College of Osteopathic Medicine, we are very excited to return to Lancaster County. I am attracted to family medicine for its variety and ability to provide full-spectrum care. Outside of medicine, my husband and I enjoy skiing, fly-fishing, hiking, cooking and spending time with our two new puppies.

Born and raised in Haiti, I convey the warmth of my lovely tropical country. I stayed there for my whole education and married my beautiful wife, the most distinguished “Creole du terroir” (native of my homeland) We also have two boys. I enjoy reading, cooking, sewing, biking and soccer playing (we are fans of Brazil, Argentina and Haiti of course). Above all, I enjoy spending time with my family--I am yet to find what is not fun when we are together. My areas of interest are public health and international medicine, HIV/AIDS and evidence-based medicine. I look forward to providing great care to the people of Lancaster County. I grew up in Lititz, Pennsylvania, completed college in Lancaster, and moved to Philadelphia for medical school. Lancaster is a great place to live, but I decided to return to the area for residency because Lancaster General offers a unique combination of unopposed training with an outstanding focus on education. I believe the best way to provide primary care is to maintain competency in a broad range of skills while keeping the patient as the focus. By knowing the patient and understanding their medical problems, the family physician becomes an indispensable part of providing safe, effective, and preventive health care. Outside of medicine, my wife and I enjoy traveling, hiking, and canoeing as well as spending time with our new daughter, Claire.

I grew up in Ridgefield, New Jersey, not far from New York City. I completed my undergraduate education at the University of Scranton in northeastern Pennsylvania where I majored in Biology and Philosophy and met my husband. I graduated from Jefferson Medical College in Philadelphia, where I was drawn to Family Medicine by its broad spectrum and emphasis on caring for the patient as a whole. Although I have lived in an urban setting for most of my life, I love the outdoors and my husband and I are very excited about beginning our married life in Lancaster County amidst so much beautiful farmland. My favorite pastimes include hiking, biking, jogging and Pilates.

Mandy R. Fannin, MD

I grew up with my roots being in various places in Pennsylvania, Virginia, and Delaware. I did my undergraduate training at Olivet Nazarene University in Kankakee, IL, where I received a degree in Biology. I took a non-traditional route and then did my Masters in Biological Science at MCP Hahnemann University (now known as Drexel Univ.). I continued at Drexel's College of Medicine for my medical education. It was during this time that I married my wonderful husband, Jimmy. My specific interests within family medicine are pediatrics and obstetrics, which attracted me to the residency program here at Lancaster General. My husband and I enjoy music (piano for me, drums for my husband), fishing, anything outdoors, and being actively involved at church.

My path to medicine has been a very nontraditional one. I was born in Israel and moved to the U.S. at the age of 3. My family settled in Philadelphia, and I knew at a young age that I wanted see more of the world. After high school I traveled the globe and discovered what was important to me, while sailing through the South Pacific, cycling in New Zealand and working on a small cruise ship in Alaska. I earned my Bachelor’s degree in the redwood oasis of Northern California, and went on to complete my medical training at Ben Gurion University in southern Israel. My passion is international medicine and women’s health. My husband and I are looking forward to learning, growing and living in Lancaster County with our daughter, Noa.

I grew up and received my medical education in China. After medical school, a desire to learn more about diseases led me to my graduate school in Shanghai and to Thomas Jefferson University in Philadelphia. With my family’s support and Jefferson’s generosity, I have been able to pursue my interests in medicine. I am interested in family care, preventive care, geriatrics, international medicine, and many more. In my spare time I enjoy spending time with my wife and daughter, hiking, traveling, swimming, and playing basketball.

Trevor Martenson, MD
Born, raised, and educated in New Mexico, I traveled a circuitous path to medicine that included working as a waiter, teaching Spanish, two years south of the border, and a master's degree in Latin American Studies. During medical school, my experiences practicing primary care with immigrant populations and on the Navajo reservation convinced me that I could do the most good, and have the most fun, as a family physician. I did my internship at a program out west but longed for the rigorous training and high professionalism that I had experienced at Lancaster as a fourth year medical student. When the opportunity arose to come back, I took it and am happy to be joining one of the top residency programs in the country. In addition to pursuing the priorities of my Christian faith, my marriage with my lovely wife Ha, and my career, I enjoy reading, writing, traveling, keeping my Spanish skills sharp, and playing with our dog, Oreo.

Anne L. Mielnik, MD

I grew up right here in beautiful Lancaster County just down the road in Ephrata, Pennsylvania. I received my Bachelor’s Degree in Liberal Arts from Villanova University and went on to study medicine at Jefferson Medical College in Philadelphia. I love Family Medicine for the opportunities it provides to care for and serve families from birth through old age. My particular interests in medicine are diverse and include women’s health, in particular Catholic alternatives in women’s healthcare and natural family planning. I am also interested in geriatrics and hospice-care. Outside of medicine, I love taking long Sunday drives through the beautiful Lancaster countryside, spending time with family, hiking, and church activities.

Yukako Tachibana, MD

I grew up in Osaka, the second largest city in Japan, also famous for the many successful comedians who are from there. I moved far west to Nagasaki to enter the Nagasaki University School of Medicine. During my medical school days, I was fortunate to be able to participate in some brief clinical experiences in the U.S. That experience stirred my interests in Family Medicine. I enjoy the broad and deep aspects of medicine as a family physician and am especially interested in maternal and child health care. Other than medicine, I love travel and movies. Both the places I go and the movies I see open my eyes toward the diversity of people, cultures, histories and their love for mankind.

I grew up in Wichita, Kansas, and then went to undergrad at York College in York, Nebraska. There, I met my husband, who grew up in Dillsburg, PA. After completing medical school at the University of Kansas we decided it was time to move back east and be close to my husband’s family. We are thrilled to be moving to Lancaster and experience the joys of east-coast living. My medical interests are diverse and include Women’s health, underserved communities, procedures and international health. Outside of medicine, my husband and I enjoy reading, hiking, traveling, and spending time with friends and family. |
